Abstract
Undoubtedly, H. pylori is the major human gastric pathogen, which infects the mucosal epithelium in 50% of world population. However, H. pylori infection is relatively prevalent globally the majority of infected individuals are asymptomatic. The recent epidemiological studies show that the various gastro intestinal complications can be the result of genetic variation in H. pylori strains. To date, different virulence factors had been suggested for H. pylori, but none of them can be a good biomarker for specific gastric disorders. In this review article, we aim to describe a comprehensive view on what we found in H. pylori virulence research in Iran. Hopefully, in parallel with new advances in sequencing systems, we will have better overview of virulence phenomenon of H. pylori in near future.

Abstract
Background and Objective: Septi scrub is a new product based on chlorhexidine 4%, which is produced for surgical scrub in Iran. This study aimed to compare the disinfecting effect of Povidone Iodine and Septiscrub in surgical scrub.
Material and Methods: this crossover experimental study, one-week interval, was conducted to compare Povidone Iodine and Septiscrub. Three samples for microbial cultures were obtained from each participant before, shortly and 40 minutes after scrubbing.
Results: The findings showed that the effect of Povidone iodine and Septiscrub solutions was significant on microbial growth of measure (P< 0.05). In both solutions, there was a significant difference in the rate of microbial growth between before and after scrub stages (P< 0.05). But between immediately after scrub and 40 minutes after was not significant (P< 0.05).
Conclusion: given the equal effect of two solutions and lesser side effect of Septiscub , it is recommended that Septiscrub be used instead of Povidone iodine 7.5%.

Abstract
Abstract
Background and Objective: Due to the continuous increase of multidrug-resistant Acinetobacter baumannii strains around the world, decision making for an effective treatment of infections caused by this organism depends on the results of antimicrobial susceptibility tests. In this study, the validity of disk diffusion and E-test methods was assessed by their comparison with the reference method of microbroth dilution for three antibiotics of tetracycline, doxycycline and minocycline.
Methods: Total of 68 A. baumannii isolates were obtained from patients hospitalized in the burn center of Shahid Motahari Hospital in Tehran, Iran. Susceptibility of the Acinetobacter isolates was evaluated using the disk diffusion, E-test and microbroth dilution methods, according to the guidelines of Clinical and Laboratory Standards Institute.
Results: Among the isolates, 82.3% were tetracycline-resistant (with minimum inhibitory concentration 50 (MIC50) and MIC90 of 32 and more than 32 µg/ml, respectively and 41.2% were doxycycline-resistant (with MIC50 and MIC90 of 4 and more than 32 µg/ml, respectively). Minocycline, with resistance of up to 13.3% (MIC50 and MIC90 of 1 and 8 µg/ml, respectively) showed the highest antimicrobial activity against the A. baumannii isolates. Antimicrobial susceptibility of bacteria was different depending on the type of methods used. No very major error was observed in any of the methods of susceptibility testing. Overall, the level of major and minor errors in the E-test was lower than the disk diffusion method.
Conclusion: The results of this study indicate that minocycline has notably high antimicrobial activity against A. baumannii compared to other antibiotics of the tetracycline group.

Background and Objective: Lophomonas blattarum is a multi-flagellate protozoan that is commensal in hindgut of cockroaches and other insects. The protozoan can cause respiratory infection in humans. Most cases of the infections with this protozoan have been reported in China. Here, we present a case with chronic respiratory allergy caused by
L. blattarum in Golestan province, Iran.
Case Description: The case was a 37-year-old male with history of respiratory conditions and he was immunocompetent. An athlete.

Conclusion: Since we found respiratory infection in an immunocompetent individual who was also an athlete, it is necessary to study this parasite and its life cycle and transmission methods. It is also suggested to consider
L. blattarum infection and treatment with metronidazole in cases of chronic allergies, especially those that do not respond to treatment.

Background and Objectives:
Hemoglobinopathies are characterized by defects in the synthesis of globin chains of hemoglobin (Hb). The purpose of the present study was to evaluate mutations associated with
thalassemia and other
hemoglobinopathies in Masjed Soleiman County, Iran.
Methods: This descriptive study was carried out on 456 individuals suspected of having hemoglobinopathies who were referred to health centers of the Masjed Soleiman Country in 2015-2017. Blood samples were collected in EDTA tubes. Complete blood count test was performed and red blood cell indices were determined. Level of Hb variants was measured using capillary electrophoresis. Reverse dot-blot, gap-polymerase chain reaction and Sanger sequencing were carried out to detect mutations.
Results: We found that 17.7% of the subjects were heterozygous for β-thalassemia. Frequency of mutations 36
/37 (–T), IVS-II-1 (G
>A) and IVS-I-110 (G
>A)
in the β-globin gene was 26.7%, 22% and 16.27%, respectively. In addition,
9.5% of the subjects contained Hb S, Hb D and Hb C, while 1.1% of the subjects showed co-inheritance of an Hb variant and β-thalassemia. In subjects with α-thalassemia, the -α3.7 (57.1%), --
MED–(17.4%), -α
4.2 (3.1%) and -
α20.5 (1.5%) deletions were found as the most prevalent mutations.
Conclusion: In addition to the high prevalence of β-thalassemia and HBB gene mutations, we detected variants Hb S, Hb D, Hb C and co-inheritance of an Hb variants and β-thalassemia in individuals living in the Masjed Soleiman Country. We also identified four mutations in the α-globin gene. These results can be useful for genetic counseling in this population.

Abstract
Background and objectives: Bone-related osteokines play an important role in the response of bone metabolism to physical activity. The purpose of this study was to investigate effects of different intensities of resistance training on serum levels of some osteokines associated with the Wnt signaling pathway and receptor activator of nuclear factor kappa-Β ligand (RANKL) pathway in old male Wistar rats.
Methods: Twenty-four old (23 months) male Wistar rats (mean weight: 437.93±33 g) were randomly divided into three groups of moderate-intensity resistance training (n=8), high-intensity resistance training (n=8) and control (n=8). Moderate- and high-intensity resistance trainings were performed at 60% and 80% maximal voluntary carrying capacity, respectively. The level of wclerostin, osteoprotegerin (OPG) and RANKL was evaluated by enzyme linked immunosorbent assay. Data were analyzed using one-way analysis of variance and at significance level of ≤0.05.
Results: The results showed that sclerostin (p=0.014), OPG (p=0.049) and RANKL (P=0.034) differed significantly between the study groups. The Tukey post-hoc comparison showed that sclerostin decreased significantly in the high-intensity resistance training group compared to moderate-intensity resistance training group (P=0.048). In addition, OPG decreased significantly in the moderate-intensity resistance training group compared to the control group (P=0.033). Moreover, RANKL decreased significantly in the high-intensity resistance training group compared to the control (P=0.048).
Conclusion: The results showed that resistance training with appropriate repetition and intensity can have positive effects on bone formation signaling pathways.

Abstract
Background and objectives: Vascular senescence refers to specific aging-associated changes in the endothelial cells. The purpose of this study was to investigate effects of 12 weeks of resistance training with elastic bands on circulating endothelial cell-derived microRNA-92a and endothelin-1 (ET-1) in elderly women with osteosarcopenic obesity (OSO).
Methods: In the present randomized clinical trial, 48 elderly women with OSO were randomly divided into a control group (n=22) and an experimental group (n=26). The subjects in the intervention group performed 12 weeks of resistance training with elastic bands three times a week. The participants became familiar with targeted number of repetitions and OMNI-resistance exercise scale to control exercise intensity.
Results: The mean age, body fat percentage, body mass index (BMI), and bone mineral density T-score for the hip and L1-L4 lumbar spine of the subjects were 64.13±3.68 years, 45.4%±6.56, 33.1±3.71 kg/m2, and -1.86±1.42, respectively. After the 12-week resistance training, we observed a significant decrease in serum level of mir-92a in the experimental group compared to the control group (p=0.03). However, there was no significant difference between the study groups in terms of body weight, BMI, body fat percentage, total cholesterol, and ET-1 (p≥0.05).
Conclusion: The 12-week resistance training program seems to modulate and decrease serum mir-92a expression in elderly women with OSO. The lack of a significant change in BMI, body fat percentage, and ET-1 levels following the exercise training might be due to the type and intensity of the exercises.

Abstract
Background and objectives: Cutaneous leishmaniasis is endemic in most areas of Iran, and the diagnosis of its species is essential for controlling the disease. Leishmania major is the causative agent for cutaneous leishmaniasis in humans. Molecular methods are generally more sensitive than microscopic methods. The present study aimed to use a polymerase chain reaction-enzyme linked immunosorbent assay (PCR-ELISA) technique for detecting live L. major from wounds of patients with cutaneous leishmaniasis.
Methods: In the present study, a standard strain of L. major promastigotes was used as the positive control for purification of DNA. The Novy–MacNeal–Nicolle and RPMI-1640 media were used for reproduction of parasites. DNA was isolated from specimens taken from 35 patients with suspected cutaneous leishmaniasis whose disease was confirmed by direct smear method. The PCR-ELISA technique was later applied by using the standard strain, patient specimens, and primers specific for the 18s rRNA.
Results: Out of 35 patients, 17 (48.6%) were male and 18 (51.4%) were female. In addition, 8.6% of the patients lived in the Gonbad-e Kavus County, while all patients had been infected in villages around Gonbad-e Kavus. Of 35 patients with confirmed cutaneous leishmaniasis according to the direct smear method, 31 patients (86.31%) had leishmaniasis based on the PCR method and the PCR-ELISA methods.
Conclusion: Based on the results, the PCR-ELISA method is more sensitive and accurate for detecting L. major.

Abstract
Background and objectives: Considering racial/ethnic differences is necessary when recommending lifestyle modifications for patients with diabetes. Racial/ethnic diversity may affect hypoadiponectinemia responses to exercise training among individuals with type 2 diabetes. This systematic review and meta-analysis is the first to investigate effects of exercise training on circulating adiponectin concentrations in Iranians with type 2 diabetes.
Methods: Literature searches of the Cochrane Central Register of Controlled Trials were carried out using the following search strategy: [exercise OR training OR physical activity OR Training] AND diabetes AND adiponectin. Next, RCTs were included and compared with each type of supervised exercise (aerobic training, resistance training, or combined training). Pooled intervention effects were evaluated and reported as standardized mean difference (MD) and 95% confidence intervals using a random effects model. Subgroup and sensitivity analysis was performed for study heterogeneity and following primary screening full text of the articles was evaluated.
Results: Fourteen studies with 444 individuals (236 men and 128 women) were included in the analysis. The age of the participants ranged between 18 and 60 years. The number of exercise sessions per week ranged between 3 and 5. The duration of interventions ranged between 6 and 12 weeks. The meta-analysis showed that adiponectin levels increased significantly in diabetic subjects after physical activity (MD: 0.72 ng/dl, p<0.001,), but the heterogeneity of the study remained significant (I2= 89%).
Conclusion: Overall, physical exercise, particularly aerobic exercise, increases adiponectin levels in Iranians with diabetes. However, this effect of exercise may be influenced by race/ethnic differences, type of training, frequency, type of adiponectin measurement, and complex and heterogeneous exercise responses of individual with diabetes.

Background: Research has shown that the extract of some plants has an effective protective effect on liver cells against toxins and oxidants. In this research, the effect of hydroalcoholic extract of ginger against hepatotoxicity caused by doxorubicin (Dox) in adult male rats was investigated.
Methods: In this experimental study, 63 adult male rats were divided into 7 groups (n = 9 per group). The experimental treatments included the control and placebo groups without the use of medicines, experimental group I that received Dox at a dose of 20 mg/kg, experimental groups II and III with 300 and 600 mg/kg of ginger hydroalcoholic extract, and experimental groups IV and V that received 20 mg/kg Dox + 300 and 600 mg/kg of ginger hydroalcoholic extract, respectively. After the end of the test period, serum levels of total bilirubin and direct bilirubin were measured. In addition, the histological changes in the liver were examined after hematoxylin and eosin (H&E) staining.
Results: Serum levels of total bilirubin and direct bilirubin in the Dox group showed a significant increase compared to the control group. In contrast, serum levels of total bilirubin and direct bilirubin in Dox + ZIN 600 and Dox + ZIN 300 had a significant reduction compared to the Dox group (P < 0.05). Ginger extract prevented apoptosis and Dox-induced liver tissue damage in dose-dependent designs.
Conclusion: The hydroalcoholic extract of ginger improves the changes of serum bilirubin and liver tissue after receiving Dox due to its antioxidant compounds.
