Abstract
ABSTRACT
Background and Objectives: The present study aimed to investigate effects of eight weeks of aquatic exercise and resistance training on plasma neurotrophin-4 (NT-4) levels and NT-4 expression in peripheral blood mononuclear cells (PBMCs) of women with multiple sclerosis (MS).
Methods: Thirty women with MS were randomly assigned to a resistance-training group, an aquatic exercise group and a control group. After separating plasma and PBMCs from blood samples, level of plasma NT-4 and NT-4 expression was measured after eight weeks of exercise via ELISA and Real-time PCR, respectively.
Results: The level of plasma NT-4 and NT-4 expression increased significantly following eight weeks of aquatic training and resistance training.
Conclusion: Based on the results of the present study, both resistance training and aquatic exercise can increase the level of plasma NT-4 and NT-4 expression in female MS patients. It can be concluded that such trainings could have protective and positive effects on the nervous system of MS patients.

Abstract
ABSTRACT
Background and Objectives: Leptospirosis is a widespread zoonotic disease that is transmitted directly or indirectly from animals to humans. Humans mainly acquire pathogenic leptospires through mucosal or percutaneous exposure to environment contaminated with urine from an infected animal. We aimed to identify pathogenic leptospiral serovars by detection of the ompL37 gene using polymerase chain reaction (PCR).
Methods: Sixteen pathogenic leptospiral serovars and a saprophytic serovar, L. biflexa were cultured in modified semisolid Ellinghausen-McCullough-Johnson-Harris medium containing 5% rabbit serum. Genomic DNA extraction was done using the phenol-chlorophorm method. The ompL37 gene was amplified using specific primers. PCR products were analyzed by agarose gel electrophoresis.
Results: The ompL37 gene was amplified only in the pathogenic leptospiral serovars. We detected no amplified fragment for the saprophytic serovar.
Conclusion: Leptospirosis may be confused with other infectious diseases, and therefore, its early and accurate diagnosis is crucial. We showed that molecular detection of pathogenic leptospires based on the ompL37 gene could be used for laboratory diagnosis of leptospirosis.

Abstract
ABSTRACT
Background and Objectives: Prostate cancer is a highly prevalent malignancy with a high mortality rate in men. Many studies have investigated the diagnostic value of various genes involved in prostate cancer, but there is no data for Kallikrein 2 (KLK2) and Kallikrein 3 (KLK3) expression patterns among Iranian patients. Therefore, we aimed to evaluate the expression of these two genes in Iranian patients with prostate cancer.
Methods: In this case-control study, three groups consisting of healthy individuals, patients with benign prostatic hyperplasia (BPH) and patients with prostate cancer were studied. Peripheral blood samples were collected from all subjects, mRNA was extracted after cell lysis, and cDNA was synthesized. Real-time PCR was performed to assess gene expression levels relative to a reference gene (18s rRNA gene).
Results: The KLK2 gene was overexpressed in patients with prostate cancer. KLK2 expression differed significantly between the cancer patients and controls. Relative expression of the KLK3 gene in the BPH group was higher than that in the control and cancer groups. However, we observed no significant difference in the expression of KLK3 between the control and cancer subjects.
Conclusion: We demonstrate that analysis of KLK2 expression is a favorable method of diagnosing prostate cancer and discriminating normal individuals from those with BPH or prostate cancer. We also found that the KLK3 gene is highly overexpressed in individuals with BPH, which might indicate that this gene is not cancer-specific.

Background and Objectives: Enterococcus faecalis is a major cause of bacterial prostatitis, which can increase the risk of developing prostate cancer if mistreated or left untreated. The aim of this study was to evaluate resistance of
E. faecalis strains isolated from patients with prostatitis to three
fluoroquinolones.
Methods: In this cross-sectional study, we collected urine specimen from 164 patients hospitalized in six hospitals in the Golestan Province, Iran. Biochemical and bacteriological tests were carried out to identify
E. faecalis strains. Pattern of resistance to ciprofloxacin, levofloxacin and norfloxacin was studied using the agar disk diffusion method (Kirby-Bauer method). The broth microdilution test was performed to determine minimum inhibitory concentrations (MICs) of fluoroquinolones according to the CLSI
M100-S25 (2015) criteria.
Results: Of 164 isolates, 39 (23.8%) were identified as
E. faecalis. Frequency of resistance to ciprofloxacin, norfloxacin and levofloxacin was 12.8%, 12.8% and 2.6%, respectively. The MIC
90 of ciprofloxacin against the isolates was
4 μg/ml, which was 4-fold lower than that of norfloxacin
(MIC90=16μg/ml) and 2-fold lower than that of levofloxacin (MIC
90=8μg/ml). We found no significant difference between the isolates in terms of resistant to the fluoroquinolones (P>0.01).
Conclusion: Our results show that
E. faecalis is one of the most common causes of bacterial prostatitis, and fluoroquinolones are still effective for treating the infection despite the reports of fluoroquinolones resistance in Iran. Moreover, levofloxacin may be a more suitable and effective antibiotic than ciprofloxacin and norfloxacin for treatment of this infection.

Abstract
Background and objectives: After using of trivalent oral poliovirus vaccine (OPV) for a long time in Iran, in recent years, due to the eradication of type 2 of poliovirus, a bivalent vaccine containing types 1 and 3 is used. Since it is a highly thermo labile vaccine strain of poliovirus, it should be stored under the recommended temperature. Because in vaccine storage and transportation, the cold chain may not be achieved in Iran tropical weather conditions, the potency of this vaccine may be subject to change.
Methods: The purpose of this study was the stability evaluation of bivalent oral poliovirus vaccine produced by Razi Institute in Iran for stability study of vaccines to ensure constancy of virus at different temperatures and intervals freezing/defrosting cycles (stress tests). Three consecutive batches produced during full-scale production were sampled randomly. All quality control tests in the form of stability studies were done on vaccines stored at -20, 2-8, 22-25 and 35-37ºC in specific time intervals and 10, 20, 30, 40 and 50 freezing/defrosting cycles.
Results: The findings indicate the stability of this vaccine in different situations. The results also showed the effects of the environment affecting factors on the potency of the vaccines and determined the correlation between the VVM grade and the vaccine potency.
Conclusion: Important observations of the vaccine distribution network at different transit levels and the correlation of VVM grade and vaccine potency and freezing/defrosting are discussed in this study which will be of help to the vaccine program team.

Abstract
ABSTRACT
Reports have shown that lipoprotein (Lp)(a) can serve as an indicator of atherosclerosis and cardiovascular diseases. Several cardiovascular disease risk factors including age, ethnicity and type 2 diabetes mellitus have been linked to Lp(a) metabolism. Given the structural similarity between Lp(a) and plasminogen, there may be a relationship between Lp(a) level and thrombosis and atherogenesis.
In this review, we summarize the latest data about Lp(a) and related conditions on the PubMed database using the following keywords: “Lp(a) and diseases” and “Lp(a) and racial groups”. All available information was extracted and categorized according to the purpose of this study. In conclusion, evidence suggest that increased level of Lp(a) results in coronary artery disease and increases the risk of ischemic stroke. Lack of Lp(a) has no adverse effect on human health. Moreover, Lp(a) can be effective in wound healing as it degrades apolipoprotein(a) products which might have anti-tumor and anti-angiogenetic effects.

Abstract
Background and objectives: Leishmaniasis is a tropical disease caused by protozoan parasites from the genus Leishmania. In this study, we aimed at investigating the in vitro anti-leishmanial effect of essential oils of Rosmarinus officinalis, Mentha pulegium, Foeniculum vulgare, Lippia citriodora and Pelargonium graveolens.
Methods: The essential oils were prepared from freshly dried and powdered plants with steam-distilled water. Iranian strain of Leishmania promastigotes was cultured in RPMI medium and the inhibitory effects of different concentrations (25, 32, 62.5, 125, 250, 500 and 1000 μg/ml) of the essential oils were investigated at 24, 48 and 72 hours. The number of live parasites before and after treatment with the essential oils was counted by trypan blue 10% staining and using neobar lam.
Results: The essential oils significantly decreased the number of promastigotes in a dose-dependent manner (P<0.05). However, the inhibitory effects of F. vulgare and R. officinalis essential oils were more profound compared to other essential oils. Moreover, concentrations of 500 and 1000 μg/ml of these two essential oils exerted equal and more anti-leishmanial potency compared to glucantime, the first-line drug used for treatment of leishmaniasis.
Conclusion: Based on the results, it is recommended to evaluate the in vivo anti-leishmanial effects of the tested essential oils, particularly F. vulgare and R. officinalis.

Abstract
Background & Objective: Cerebral ischemia causes irreversible structural and functional damages in certain areas of the brain, especially the hippocampus. Evidence indicates that physical exercise may reduce the damages caused by cerebral ischemia. The purpose of this study was to examine the effect of 8-week exercise preconditioning on the expression of NT-3 and TrkC genes in the CA1 region of the hippocampus after the cerebral ischemic-reperfusion in male rats.
Methods: Twenty-one male Wistar rats weighing 250-300 gr were randomly selected and divided into three groups (healthy control, control + ischemia and exercise + ischemia). Rats in the exercise group ran on a treadmill 5 days per week for 8 weeks. Ischemia by occlusion of both common carotid arteries (CCA) was created for 45 minutes. In order to evaluate the gene expression, Real time PCR technique was used.
Findings: NT-3 gene expression was significantly different between exercise + ischemia with control + ischemia groups and control + ischemia with healthy control groups (P <0.05), and TrkC gene expression was significantly different between exercise + ischemia with healthy control groups and control + ischemia with healthy control groups (P <0.05).
Conclusion: The results of this study demonstrated that exercise before the induction of ischemic stroke increased the NT-3 gene expression but did not influence the TrKC gene expression.

Abstract
Leptospirosis is a zoonotic disease with a high incidence rate in many parts of the world due to the presence of various hosts for the pathogenic Leptospira. Tropical, subtropical and humid regions are suitable for long-term survival of the bacterium. Because of the temperate and humid climate, northern areas of Iran are suitable for pathogenic Leptospira and outbreak of the disease. Therefore, identification of infected areas is important from a public health and economic point of view. Previous studies show that the incidence rate of leptospirosis is increasing every year. Therefore, accurate diagnosis, control and prevention of this disease seem necessary through vaccination and raising public awareness, especially among high-risk groups. Today, diagnostic methods including immunofluorescence assay, enzyme-linked immunosorbent assay, microscopic agglutination test (MAT) and polymerase chain reaction (PCR) are used to diagnose the leptospirosis. MAT is the gold standard test for the diagnosis of leptospirosis with extensive applications in Iran. Due to the importance of this disease and its high prevalence in recent years, the present study aimed to investigate the epidemiology and diagnosis of leptospirosis in Iran.

Abstract
Background and objectives: Multiple sclerosis (MS) is a common debilitating neurodegenerative disease caused by inflammatory demyelinating processes in the central nervous system. The present study aimed to evaluate the effects of core stability training on serum levels of progranulin and tumor necrosis factor alpha (TNF-α) in women with MS.
Methods: The study population consisted of 24 women with MS (aged 25 to 40 years) in Bojnourd (Iran) with expanded disability status scale score of 2-5. Patients were randomly divided into two groups of core stability training (n=12) and control (n=12). The training group performed 30-40 minutes of core stability training, three sessions a week for eight weeks. Blood sampling was done 24 hours before the first session and 48 hours after the last training session. Serum levels of progranulin and TNF-α were measured using commercial ELISA kits. Analysis of covariance was used to assess data at a significance level of 0.05 .
Results: The eight-week core stability training was associated with a significant increase in serum level of progranulin (P=0.037) and a significant decrease in the level of TNF-α (P=0.000).
Conclusion: The findings indicate that the eight-week core stability training could significantly change serum levels of progranulin and TNF-α and improve the condition of women with MS. Therefore, this type of training could be applied as a complementary therapy for MS patients.

Abstract
Background and objectives: Multiple sclerosis (MS) is a demyelinating and neurodegenerative disease of the centralnervous system that can be tracked through biomarkers of disease status. The present study aimed to examine effect of eight weeks of resistance training on serum levels of neurofilament light chain and tau protein in women with multiple sclerosis (MS).
Methods: The study population consisted of 24 women with MS (aged 25 to 40 years) in Bojnourd (Iran) with expanded disability status scale score of 2-5. Patients were randomly divided into two groups of resistance training (n=12) and control (n=12). The training group performed 45-60 minutes of resistance training, three sessions a week for eight weeks. The control group did not partake in sports activity. Blood samples were taken 24 hours before the first session and 48 hours after the last training session. Analysis of covariance was used to analyze data at a significance level of 0.05.
Results: The eight-week resistance training intervention significantly decreased serum level of tau protein but had no significant effect on serum level of neurofilamet light chain.
Conclusion: According to the research results, eight weeks of resistance training could have favorable effects on serum level of tau protein in MS patients.

Abstract
Background and objectives: Microscopic agglutination test is the gold standard sero-diagnostic method for detection of leptospirosis. Moreover, it helps identify serovars and their titers in serum samples. For obtaining accurate titer results, proper sampling, collection, storage, and transportation of samples are crucial while maintaining the cold chain. Since storage for long periods and the subsequent deterioration of samples may affect the final titers, we proposed an alternative method of MAT testing using filter paper-dried serum samples. We also evaluated sensitivity and specificity of the MAT test by using filtered-dried serum samples compared with the conventional MAT test.
Methods: This experimental study was performed on human and animal serum samples that were sent to a reference leprospirosis laboratory in 2020. Overall, 142 positive samples (with 289 titers for different strains) and 15 negative samples were used for MAT test using filtered-dried serum. For this purpose, each sample was dried on a filter paper (Whatman 903, GE Healthcare) at room temperature (20-30 °C) and kept for four days. On the fifth day, the filter papers were cut into small pieces, soaked in phosphate buffer saline, vortexed, and slowly mixed on shaker for two hours to elute antibodies. The MAT tests were performed simultaneously and under the same environmental conditions.
Results: The new MAT test using dried serum samples showed 79% sensitivity and 100% specificity. The test also had positive predictive value of 92% and negative predictive value of 24% when compared with the gold standard MAT test.
Conclusion: Filter-dried serum can be used for MAT test to overcome serum storage and transportation problems.

Abstract
Background and objectives: Vascular senescence refers to specific aging-associated changes in the endothelial cells. The purpose of this study was to investigate effects of 12 weeks of resistance training with elastic bands on circulating endothelial cell-derived microRNA-92a and endothelin-1 (ET-1) in elderly women with osteosarcopenic obesity (OSO).
Methods: In the present randomized clinical trial, 48 elderly women with OSO were randomly divided into a control group (n=22) and an experimental group (n=26). The subjects in the intervention group performed 12 weeks of resistance training with elastic bands three times a week. The participants became familiar with targeted number of repetitions and OMNI-resistance exercise scale to control exercise intensity.
Results: The mean age, body fat percentage, body mass index (BMI), and bone mineral density T-score for the hip and L1-L4 lumbar spine of the subjects were 64.13±3.68 years, 45.4%±6.56, 33.1±3.71 kg/m2, and -1.86±1.42, respectively. After the 12-week resistance training, we observed a significant decrease in serum level of mir-92a in the experimental group compared to the control group (p=0.03). However, there was no significant difference between the study groups in terms of body weight, BMI, body fat percentage, total cholesterol, and ET-1 (p≥0.05).
Conclusion: The 12-week resistance training program seems to modulate and decrease serum mir-92a expression in elderly women with OSO. The lack of a significant change in BMI, body fat percentage, and ET-1 levels following the exercise training might be due to the type and intensity of the exercises.

Abstract
Background and objectives: Coronavirus disease 2019 (COVID-19) is a communicable disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The clinical manifestations of COVID-19 vary from asymptomatic to acute respiratory distress syndrome. In severely-ill patients, neutrophil count is significantly increased. This study aimed to evaluate absolute neutrophil count (ANC) in patients with COVID-19 and healthy individuals.
Methods: This retrospective, cross-sectional study was carried out on patients admitted to the outpatient department of OBGY Rural Hospital (Maharashtra, India) from April 2021 to June 2021. A peripheral venous blood sample (3 ml) was taken from 143 patients with COVID-19 and 143 healthy controls. The blood samples were analyzed using a fully automated blood cell counter for the estimation of complete blood count. A peripheral blood smear was prepared and stained with Leishman stain for evaluation of morphological changes.
Results: The ANC of COVID-19 patients was significantly higher than that of healthy control subjects (p=0.00001). Toxic granules (48.9%) were the most common morphological neutrophil abnormality, followed by hypolobation, hypogranulation, Pseudo-Pelger-Huet anomaly (20.9%), and shift to left with the presence of immature myeloid series cells.
Conclusion: High ANC along with morphological changes in neutrophils could be used as a simple and inexpensive surrogate marker of COVID-19 and its severity.

Abstract
Background and objectives: Osteoporosis is a disease characterized by low bone mass and microarchitectural deterioration of bone tissue. It is the most common chronic metabolic bone disease. The purpose of this study was to compare the level of bone metabolic markers including parathyroid hormone (PTH), alkaline phosphatase (ALP), calcium, and phosphorus between female basketball players and non-athlete females.
Methods: Twelve female professional basketball players (aged 20-35 years) of the Super League of Golestan Province (Iran) were enrolled as the experimental group. Fifteen age-matched non-athlete females were also selected as the control group. Plasma levels of PTH, ALP, calcium, and phosphorus were compared between the study groups.
Results: The levels of PTH (p=0.004) and ALP (p=0.001) were significantly higher in the experimental group than in the control group. The levels of calcium and phosphorus did not differ significantly between the study groups (p>0.05).
Conclusion: Based on the results, it could be stated that performing weight-bearing sports activities such as basketball can improve the density of bone minerals and the factors stimulating bone formation.

Abstract
Background and objectives: Granulomatous disorders of the skin are frequently encountered in clinical practice and require histopathological confirmation due to a considerable etiological and clinical overlap. A single histopathological pattern may be produced by many causative agents and at the same time, a single cause can present with varied histopathological patterns. The present study was performed to evaluate the histomorphological patterns of granulomas in various granulomatous skin lesions and to identify the causative agents.
Methods: The study (both prospective and retrospective) was carried out in the department of pathology over 5 years. All skin biopsies were evaluated for the presence of granulomas. Detailed analysis of the histopathological pattern of granulomas was performed and categorization was made according to the type and etiology. Special stains were also used when required. A clinicopathological correlation was established with the Kappa statistic.
Results: Of 1,150 skin biopsies, granulomatous skin lesions were observed in 325 cases. Histiocytic granuloma pattern was the most common subtype (55.7%). The predominant etiology of granulomatous inflammation was leprosy (93.5%), followed by cutaneous tuberculosis (2.7%). The cases of Hansen’s disease showed a maximum clinicopathological correlation (58.5%).
Conclusion: Histopathological examination is the gold standard for the diagnosis and subtyping of granulomatous skin lesions. Varied morphologies of granuloma patterns were observed in our study, and infectious diseases were the causative agents in the majority of cutaneous granulomatous disorders.

Abstract
Background and objectives: Endometriosis is a chronic disease that affects approximately 10% of women of reproductive age. The purpose of this study was to evaluate the effects of 8 weeks of swimming exercise and omega-3 supplementation on the expression of cyclooxygenase-2 (COX-2) and prostaglandin E2 (PGE2) genes and serum levels of reproductive hormones in the endometrial tissue of a rat model of endometriosis.
Methods: In this experimental study, 30 adult Wistar rats were randomly divided into 5 groups of healthy-control, patient-control, patient+exercise, patient+omega-3, and patient+omega-3+exercise groups. After the induction of endometriosis, the rats were subjected to 8 weeks of swimming exercise, 5 days a week as well as 2 ml/kg/body weight of omega-3. Data were analyzed using one-way analysis of variance and Tukey post hoc test at the significance level of 0.05.
Results: The expression of COX-2 and PGE2 as well as serum follicle-stimulating hormone (FSH), luteinizing hormone (LH), and estradiol levels were significantly higher in the patient group compared with the healthy-control group (p≤0.0001). Exercise and omega-3 supplementation either separately or combined could significantly reduce the expression of both genes and serum levels of LH, FSH, and estradiol (p<0.05). This effect was more profound in the patient+exercise+omega-3 group.
Conclusion: The results of the present study indicate that regulating the expression of COX-2 and PGE2 genes as well as serum levels of reproductive hormones through swimming exercise and omega-3 supplementation can improve endometriosis.

Abstract
Background and objectives: Studies have reported contradictory results regarding the relationship between serum neurofilament light chain (NFL) and fatigue severity. The aim of our study was to investigate serum NFL and fatigue severity in patients with multiple sclerosis (MS) after home-based and outdoor Pilates training.
Methods: The study population consisted of 44 women with MS (aged 25 to 40 years) with an expanded disability status scale score of 2-5. The patients were randomly divided into three groups: home-based training (HPT, n=15), outdoor training (OPT, n=15), and a control group (n=14). The training groups performed Pilates exercises for 8 weeks individually at home and under the sun. Blood samples and completed Fatigue Severity Scale forms were taken 48 hours before the first session and after the 8-week intervention.
Results: The results showed that OPT caused a more notable increase in serum vitamin D (32.76±5.13, p<0.001) and a decrease in NFL (15.98±2.26, p <0.001) and FSS score (2.51±0.35, p<0.001) compared with HPT. Also, HPT lowered NFL (18.01±2.28, p<0.001) and FSS score (3.84±0.98, p<0.001) compared with the control group.
Conclusion: Pilates training can reduce NFL levels and FSS scores in women with MS. In addition, OPT is more effective than HPT in reducing NFL and FSS and increasing vitamin D levels.

Background: This study investigated the impact of a 3-week detraining period, followed by 10 weeks of aerobic training, on the serum levels of osteocalcin and alkaline phosphatase (ALP) in asthmatic males.
Methods: Thirty adult males with asthma were randomly divided into experimental and control groups. The experimental group engaged in a 10-week aerobic training program, which included running sessions 3 times per week for 30-60 minutes each, maintaining a target heart rate range of 55%-75% of their maximum heart rate (HRmax). In contrast, the control subjects did not participate in any exercise activities during the study period. Fasting blood samples were obtained at baseline, after aerobic training, and after detraining to measure the levels of osteocalcin and ALP in both groups.
Results: The 10-week aerobic training program led to a significant increase in levels of osteocalcin and ALP in the experimental group, but following the detraining period, these variables returned to their pre-training levels.
Conclusion: The effectiveness of continuous aerobic training on bone formation markers in asthmatic patients will disappear after a relatively short period of detraining. This evidence emphasizes the lack of stopping aerobic exercise in the presence of inhaled corticosteroids with the aim of improving bone metabolism in asthmatic patients.

Background: Multiple sclerosis (MS) is a demyelination disorder of the central nervous system (CNS), which is believed to be associated with oxidative stress. Therefore, researchers try to find reliable biomarkers to monitor the disease and predict its prognosis. Cholesterol and lipids in the myelin sheath are vital for nerve cells. Serum low-density lipoprotein (LDL) is susceptible to lipid peroxidation induced by oxidative stress. This study aimed to evaluate oxidative stress markers in the serum of patients with relapsing-remitting MS (RRMS) and examine their correlation with lipid markers.
Methods: A total of 18 MS patients (14 women and 4 men) and 18 healthy subjects (matched by age and sex) were enrolled in this cross-sectional study. The serum samples were collected in both relapsing and remitting phases. The prooxidant-antioxidant balance (PAB), malondialdehyde (MDA), and oxidized LDL (oxLDL) were measured as markers of oxidative stress.
Results: The mean age of participants was 29.21 (22-42) years. In the comparison between the patient and control groups, the most differences were increased levels of PAB in the patient group (P < 0.05), no difference between relapsing and remitting phases (P = 0.995), increased MDA levels in the relapsing phase (P = 0.013)––but no change in the remitting phase (P = 0.068), no difference in LDL and oxLDL levels in the patient group (P > 0.05), and MDA, LDL, and oxLDL levels did not have any significant correlation with PAB (P > 0.05).
Conclusion: High levels of oxidative stress markers were present in both phases of the disease. Lipid peroxidation markers (such as MDA) increased in the acute phase, but oxLDL did not change. Also, there was no significant correlation between oxidative stress and cholesterol markers.
